Overview
An exciting opportunity has arisen for a full-time Social Worker with post ASYE experience in the West Disabled Young People\'s Team. You will work closely with colleagues in Disabled Children\'s Social Work Teams and other key agencies, such as Health, Education and the voluntary sector. The team covers a large geographical area, and you will be expected to work flexibly across the area covered by the team. This post is a customer-facing position with a statutory duty to ensure appropriate command of spoken English/Welsh for the effective performance of job requirements.

Responsibilities
* Work with disabled young people aged 18 to 25 to progress towards independence.
* Engage in a wide range of work including Care Act Assessments, Mental Capacity Assessments, and Deprivation of Liberty Safeguards (DoLS).
* Support young people into adulthood and collaborate with Families, Carers, and other professionals and agencies.
* Coordinate with Senior Practitioners, Social Workers, and Social Work Assistants, and work with other teams and external partners such as Health, Education, and the voluntary sector.
* Be flexible in working across the geographical area covered by the team.
Qualifications
* Degree in Social Work (CQSW, DipSW or equivalent).
* Registered with Social Work England.
* Post-qualification experience in social work.
