We are a veryfriendly, rural practice currently operating from two sites in the villages of Burhamand Eccles and covering surrounding villages. With our growingpatient list from the residential development within our catchment area, we arelooking for an enthusiastic individual to join our stable and cohesive practiceteam.


Main duties of the job

Responsibility, care andmanagement of an allocated list of patients, undertaking all clinical andadministrative tasks as needed ensuring the highest standards of care for allregistered and temporary patients. To work at both sites of the practice as required.


About us

Additionalclinical workload support from our GP principal, 2 salaried GP's; an AdvancedNurse Practitioner; a Prescribing Paramedic; 3 Practice Nurses; 2 HCAs; MedicalPA and administration team..

PCNworkforce includes pharmacy team, First Contact Physiotherapists; Child Health& Wellbeing Navigator; Dementia Care Coordinator; Learning Disability Carecoordinator; Social prescriber and paramedic led clinical assessment team

JOB DESCRIPTION

Job Title: SalariedGeneral Practitioner

Place of Work: 33Bell Lane, Burham Rochester Kent ME1 3SX and Whitehouse Surgery: MackendersLane Eccles Aylesford, Kent ME20 7HX

Sessions & Days of Work: 4 sessions per week - Thursdays & Fridays (open to applications from the GP Retainer Scheme)

Annual leave: Sixweeks annual leave plus Bank Holidays (prorated)

Study leave: Oneweek per annum (prorated)

Salary: Negotiableand subject to qualifications and experience

Accountable to: The Principal GP

Reports to: ThePrincipal GP (Clinically)

The Practice Manager (Administratively)

Overall Purpose:

To provideNHS personal medical services to registered patients on the Practice list.

Job Summary:

Responsibility, care andmanagement of an allocated list of patients, undertaking all clinical andadministrative tasks as needed ensuring the highest standards of care for allregistered and temporary patients. To work at both sites of the practice as required.

Clinical Responsibilities:

In accordancewith the Practice timetable, as agreed, the post-holder will make themselvesavailable to undertake a variety of duties including surgery consultations,remote and telephone consultations and queries, triage, checking and signingrepeat prescriptions and dealing with queries, paperwork and correspondence ina timely fashion. We currently have external provision for home visits via ourPrimary Care Network but this may change in the future.

Respond tomedical problems presented by temporary residents as required

Makingprofessional, autonomous decisions in relation to presenting problems, whetherself-referred or referred from other health care workers within theorganisation

Assessing thehealth care needs of patients with undifferentiated and undiagnosed problems

Screeningpatients for disease risk factors and early signs of illness

Inconsultation with patients and in line with current Practice disease managementprotocols, developing care plans for health

Toassist in the establishment of appropriate systems to manage common chronicmedical conditions e.g. Asthma, COPD, Diabetes etc.

Completionof clinically related administrative and non-clinical duties needed for thedelivery of the service

Providingcounselling and health education

Referpatients for further opinions and interventions according to practice protocols

Recordingclear and contemporaneous consultation notes to agreed standards usingtemplates provided where appropriate

Compilingand issuing computer-generated acute and repeat prescriptions (avoidinghand-written prescriptions whenever possible)

Prescribingin accordance with the Practice prescribing formulary (or generically) wheneverthis is clinically appropriate

Beprepared to take part in the evening or early morning rota for improved and extendedaccess

Promotepatient participation in clinical research trials that the Practice is engagewith, where appropriate

Ingeneral the post-holder will be expected to undertake all the normal duties andresponsibilities associated with a GP working within primary care.

Maintaining,Monitoring & Updating Computer Database and Management of Medical Records& Practice Audits

Ensurethat practice clinical database is updated promptly and accurately withinformation relating to patient records, as required for clinical accuracy andfor practice recording requirements under IM&T protocols, QoF, QIPP,Enhanced Services and other clinical initiatives the practice partakes in.

Respondto alerts on records

Appropriateuse of read/SNOWMED codes (via templates where appropriate)

Participatingin the collection and collation of statistics towards the preparation ofpractice reports, performance monitoring and audits

Participationin the auditing of Practice activity

Participationin work involved for CQC compliance

PracticeOrganisation

Processand action incoming patient and hospital correspondence

Undertakeprivate medicals as required

Attendanceat primary health care team meetings, significant event audit meetings, annualcomplaint audit meeting and others as deemed necessary

Initiateand participate in the formulation and implementation of new protocols,policies and procedures to aid and improve administrative organisation and theprovision of services to patients

Ensurethat the Practice complaints system is adhered to at all times

Awarenessof and compliance with all relevant Practice policies/guidelines, e.g.prescribing, confidentiality, data protection, health and safety

Completionof Information Governance on line toolkit or attendance at group trainingannually and provide certificate

Maintaincurrent CPR and anaphylaxis certification and safeguarding for adults andchildren certificates.

Engagein appropriate educational activity to maintain clinical competence andperformance

Attendanceand participation in educational meetings in the Practice and externally
