Overall Purpose:
To provideNHS personal medical services to registered patients on the Practice list.
Job Summary:

Clinical Responsibilities:
In accordancewith the Practice timetable, as agreed, the post-holder will make themselvesavailable to undertake a variety of duties including surgery consultations,remote and telephone consultations and queries, triage, checking and signingrepeat prescriptions and dealing with queries, paperwork and correspondence ina timely fashion. We currently have external provision for home visits via ourPrimary Care Network but this may change in the future.

Makingprofessional, autonomous decisions in relation to presenting problems, whetherself-referred or referred from other health care workers within theorganisation
Assessing thehealth care needs of patients with undifferentiated and undiagnosed problems
Screeningpatients for disease risk factors and early signs of illness
Inconsultation with patients and in line with current Practice disease managementprotocols, developing care plans for health
Toassist in the establishment of appropriate systems to manage common chronicmedical conditions e.g. Asthma, COPD, Diabetes etc.
Completionof clinically related administrative and non-clinical duties needed for thedelivery of the service
Providingcounselling and health education
Referpatients for further opinions and interventions according to practice protocols
Recordingclear and contemporaneous consultation notes to agreed standards usingtemplates provided where appropriate
Compilingand issuing computer-generated acute and repeat prescriptions (avoidinghand-written prescriptions whenever possible)
Prescribingin accordance with the Practice prescribing formulary (or generically) wheneverthis is clinically appropriate
Beprepared to take part in the evening or early morning rota for improved and extendedaccess
Promotepatient participation in clinical research trials that the Practice is engagewith, where appropriate
Ingeneral the post-holder will be expected to undertake all the normal duties andresponsibilities associated with a GP working within primary care.
Maintaining,Monitoring & Updating Computer Database and Management of Medical Records& Practice Audits
Ensurethat practice clinical database is updated promptly and accurately withinformation relating to patient records, as required for clinical accuracy andfor practice recording requirements under IM&T protocols, QoF, QIPP,Enhanced Services and other clinical initiatives the practice partakes in.

Appropriateuse of read/SNOWMED codes (via templates where appropriate)
Participatingin the collection and collation of statistics towards the preparation ofpractice reports, performance monitoring and audits
