Are you ready to take on a rewarding challenge? We’re seeking a skilled and committedHealth and Wellbeing Assessorto join ourLIFE (Living Independently For Everyone)service. This role plays a vital part in supporting individuals to regain independence and live confidently at home.
As a trusted assessor, you’ll work alongside Social Workers, Occupational Therapists, Physiotherapists, and Intermediate Care Nurses. You’ll carry out delegated tasks and assessments, and at this level, you’ll also be responsible forsupervising Grade 6 staff.
You’ll work autonomously across a range of settings, including hospitals, community locations, and people’s homes delivering person-centred support that empowers individuals to reach their full potential.
