We are seeking a Social Worker to support young people and adults transitioning from children’s services to adult care. This vital role ensures individuals experience a smooth transition, fostering independence, stability, and positive outcomes.
Key Responsibilities:
* Conduct comprehensive assessments and develop tailored transition plans.
* Provide guidance and advocacy to young people and their families throughout the transition.
* Collaborate with multidisciplinary teams, including education, health, and social care professionals.
* Ensure compliance with safeguarding policies and care legislation.
* Maintain accurate records and prepare detailed reports for decision-making.
* Promote independence, inclusion, and person-centered care approaches.

* Hold a social work degree
