We are looking for a Care Navigator to join the team at Debenham Group Practice. This is a patient-facing role at the heart of how the practice runs day to day, and we are looking for someone who will bring a calm, friendly and organised approach to supporting our patients and colleagues.

Debenham Group Practice is part of Suffolk Primary Care, which gives us access to a wider range of shared expertise and organisational support, while allowing us to remain a well-regarded local practice with a strong community identity.

We care for 10,000 patients across our main site in Debenham, with branch surgeries at Otley and Grundisburgh.

We are looking for someone who:

* Is calm, warm and professional when dealing with patients and colleagues
* Listens carefully and records information accurately
* Is organised, reliable and takes their responsibilities seriously
* Works well as part of a team and is willing to support colleagues
* Has good IT skills and picks up new systems with confidence
* Has previous experience in a customer‑facing or administrative role

Experience in a healthcare or NHS setting is helpful but not essential we are happy to support the right person to develop in the role.


Main duties of the job

Care Navigators are often the first voice or face a patient encounters. Whether patients contact us by phone, walk through the door, or submit a request online, you will help make sure they get the right information and reach the right place.

We use askmyGP as our primary workflow tool for clinical requests. Rather than triaging patients themselves, Care Navigators help ensure that requests are accurately recorded so that the duty GP can review and act on them promptly. You will also take turns supporting the duty GP as part of the wider Care Navigation team, and you will help signpost patients to other local services where that is the most appropriate response.

The role also covers the full range of day‑to‑day practice administration from reception duties and managing records, to supporting clinical and dispensary colleagues and keeping the practice running smoothly.

You will work across our sites as needed, as part of a well‑established, friendly team.

We are looking to recruit for Wednesday, Thursday and Friday. Shifts fall within a working window of approximately 07:30 to 18:30 across those days. Depending on recruitment, hours may be split across more than one person.

Job responsibilities

Care Navigators are the first point of contact for patients and visitors to the practice by phone, in person and through online systems. The role is central to how the practice functions day to day and requires someone who is calm, organised and genuinely committed to helping patients access the right care.

You will provide a courteous, efficient and helpful service to patients and colleagues, support clinical and administrative teams with a range of tasks, and help ensure the smooth running of the practice across all sites.


Person Specification


Qualifications

* GCSE grade A to C in English and Maths


Experience

* Experience in a busy customer‑facing or administrative role, where dealing with people and managing competing demands was a core part of the job
* Confident using the telephone and computers and standard office equipment; able to pick up new systems and processes without difficulty
* Previous experience in an NHS or primary care setting
* Familiarity with SystmOne or a similar clinical system
* Experience of askmyGP or a similar online consultation or workflow tool
* Understanding of local health and care services and how they fit together
* Experience working in an environment where priorities shift and adaptability is required
