Job overview

An opportunity has arisen for a suitably experienced Learning Disability Nurse to join the Oldham Children's Community Learning Disability Team. The post holder will provide specialist health assessments and Positive Behavioural Support to children and young people with a learning disability who are registered with an Oldham GP.

Interventions will be delivered across different settings including educational provision and the person’s home. There will be a requirement to liaise with and work collaboratively with parents, partners and stakeholders to ensure responsive, effective, safe and child centred services are delivered.


Main duties of the job

* To work in accordance with Pennine Care NHS Foundation Trust policies and procedures at all times.
* To provide a safe, effective, responsive, efficient and timely community nursing services to children and young people with learning disabilities, their families and carers.
* To undertake a range of comprehensive, specialist assessments for children and young people with a learning disability and complex needs. This assessment work requires a specialist knowledge base on which to practice.
* The post holder may be required to undertake additional training in implementing high quality Positive Behavioural Support
* To develop a range of interventions and specialised care plans.
* To embrace the public health role, promoting health and wellbeing and where possible, prevent ill health.
* To support the work of the Community Learning Disability Team, in reducing the health inequalities faced by people with learning disabilities.
* To work collaboratively with primary care and secondary care services to ensure the health needs of people with learning disabilities are met.
* The post holder will undertake an active role in identifying and managing risk.
* The post holder will be required to work in close partnership with Social Services and Child Safeguarding.

Detailed job description and main responsibilities

To work within the Learning Disability Directorate as member of the Children’s Learning Disability Team which currently sits within in the Tameside and Glossop Learning Disability Team.

The post holder will provide specialist health and behaviour assessments to children and young people with a learning disability who are referred in to the team.

Interventions will be delivered across different settings including the person’s home. There will be a requirement to liaise with and work collaboratively with partners and stakeholders to ensure responsive, effective, safe and child centred services are delivered.

Experience


Essential criteria

* Demonstrate knowledge and experience supporting children with a learning disability
* Experienced in managing risk and working in partnership wiht all partners
* Knowledge of LD specialist assessment tools
* Ability to deal with complex sensitive and distressing situations which may include challenging behaviour,abuse and breaking bad news
* Knowledge of child safeguarding
* Ability to complete complex assessments and formulations
* Experience of supporting people with complex and challenging needs
* Knowledge and experience of Positive Behavioral Support


Desirable criteria

* Formal Training in positive behavioral approaches
* Training in clinical supervision
* Previous community nursing experience
* Knowledge of transforming care/SEND/EHCPs
* Experience in delivering training


Education


Essential criteria

* RNLD
* Post registration training / willingness to undertake training plus experience to postgraduate diploma level.


Desirable criteria

* Training in Positive behavioural approaches
* Training in Clinical Supervision/Mentors hip


Knowledge


Essential criteria

* Knowledge of Person Centred approaches
* Knowledge of using LD specialist assessment tools
* Knowledge of behavioural techniques /management
* Knowledge and skills across a range of learning disability issues
* Knowledge of Child Safeguarding and Adult Safeguarding
* Knowledge of current strategic drivers for community learning disability teams


Desirable criteria

* Knowledge of health issues that impact upon children and young people with a learning disability
* Knowledge of Transforming Care
* Knowledge of SEND and EHC


Skills and Abilities


Essential criteria

* Ability to complete complex assessments.
* Highly developed communication skills to support a range of activities
* Ability to collaborate in the completion of formulations.
* Ability to develop multi-element interventions / support packages.
* Ability to incorporate key principles of ‘Valuing People’ into direct client care.
* Ability to proactively manage competing demands.


Work Related Circumstances


Essential criteria

* A positive approach to working in what, at times, will be complex, emotionally charged and challenging work.
* Use of a car or access to a means of mobility to travel across the Trust footprint in line with service needs
