The Role
We are looking for Diabetes Nurse Specialists with a diabetes diploma to join our team in the primary care setting. The role involves providing excellent patient care and promoting self-care management for patients diagnosed with diabetes. The post holder should be highly organized and committed to maintaining high standards in quality and patient care.
Responsibilities include:
1. Working autonomously to deliver high-quality, evidence-based diabetes care.
2. Promoting self-care management to prevent condition worsening and improve health outcomes.
3. Ensuring the delivery of best practices in clinical care, caseload management, education, research, and audit to meet local population objectives.
4. Serving as a specialist resource and providing advice related to diabetes to patients, families, carers, and healthcare professionals.
Westrop Medical Practice serves a population of over 48,000 across six locations, including semi-rural areas near the Cotswolds, Old Town, and North Swindon.
