Overview
We have a fixed term Band 6 Community Psychiatric Nurse role in the Lincoln Community Mental Health Team at Lincolnshire Partnership NHS Foundation Trust (LPFT NHS). We are looking for a motivated and enthusiastic RMN to join our community team in Lincoln. Lincoln’s Adult Community Mental Health Services provide quality recovery-based support to people with severe and enduring mental health related difficulties. The post holder will work within a friendly Multi-Disciplinary Team including Community Mental Health Nurses, Recovery Practitioners, Occupational Therapists, Nursing Associates, Community Support Workers, Psychiatrists, Psychologists and Admin staff, and alongside Neighbourhood Teams, Integrated Placement Based Teams, Adult Social Care, GP surgeries and other partner organisations to ensure the best outcomes for patients and carers.
Responsibilities
* Provide high quality person-centred care to patients, families and carers.
* Act as care co-ordinator, undertaking comprehensive evidence-based assessments.
* Use sound knowledge of psycho-social interventions to support service users to work towards recovery goals, build self-resilience and promote independent living.
* Work within a multi-disciplinary team and collaborate with partner organisations to achieve the best outcomes for patients and carers.
* Receive robust clinical and managerial supervision regularly and participate in ongoing professional development to maintain up-to-date evidence-based practice.
Working arrangements
* The post requires travel in the community to deliver services with a hybrid approach to work.
Requirements / Qualifications
* Band 6 Community Psychiatric Nurse role; motivated and enthusiastic RMN.
* Experience in community mental health settings and delivering evidence-based psychosocial interventions.
* Ability to work within a multi-disciplinary team and with partner organisations.
* Willingness to participate in supervision and professional development; access to additional training.
* Travel in the community to deliver services with a hybrid working arrangement.
