About the Role
Are you a qualified mental health nurse ready to make a real difference in people's lives? As a Band 6 Care Coordinator within Southwark Community Mental Health Services, you’ll play a key role in supporting individuals on their journey to recovery. This role involves leading care plans, delivering targeted interventions, and helping service users overcome barriers and build on their strengths.
Key Responsibilities
* Manage your own caseload and collaborate with a multidisciplinary team.
* Develop recovery‑focused solutions tailored to each individual’s needs.
* Empower service users to take ownership of their care while ensuring they have access to the resources and support they need to thrive.
* Lead clinical discussions and support the pathway out of the CMHT.
* Act as an interface between partner agencies to ensure timely and effective transition and discharge into the wider community.
* Maintain up‑to‑date documentation and relevant assessments in patient electronic records.
* Provide support to carers and manage complex needs and varying diagnoses.
* Support the client’s journey during their time under CMHT care and contribute positively in team meetings and on duty.
Qualifications & Experience
* Clinical experience and strong communication skills.
* Passion for working collaboratively to promote independence and wellbeing.
* Significant knowledge, awareness and understanding of Mental Health Law, including the Community Care Act (2015), the Mental Capacity Act (2005) and Community Treatment Orders.
* Personal responsibility and professional accountability for a caseload as part of the community team.
* Ability to lead and manage the work of others as required.
