Paediatric Complex Discharge Nurse

We are looking to appoint an experienced paediatric nurse to our Complex Discharge post. You should have extensive experience caring for children with complex needs, ideally in both acute and community settings. You will bring sound clinical skills, expertise across a wide breadth of nursing, and the ability to provide leadership as a key worker to achieve excellence in care and discharge planning. The overriding focus will be on quality of life for children and young people, with a commitment to inclusivity when making plans. You will work with specialist multidisciplinary teams and act as a point of contact for children and young people returning to their homes, specialist rehabilitation centres, or their local hospital. Outstanding communication and organisational skills are essential, as is the ability to support staff through teaching, training, and specialist advice.


Responsibilities

* Provide leadership and act as a key worker to achieve excellence in care and discharge planning.
* Deliver the key NSF standards for children with complex needs.
* Focus on quality of life and care for children and young people, ensuring inclusive planning.
* Seek the best care solutions within agreed timeframes, challenging view on patient/carer risks and resilience where appropriate.
* Work across professional systems and geographic boundaries to facilitate the delivery of key objectives.
* Provide specialist advice, support and education to staff and students.
* Implement a clearly defined integrated approach to patient care, incorporating new treatments and evidence‑based care.
* Undertake research and lead clinical audits in your specialist area.
* Support inpatient areas to facilitate smooth patient journeys and avoid delays in discharge.

Eligibility and Immigration

Due to Home Office immigration rules, this full‑time permanent vacancy cannot be filled by individuals on a Student visa unless they have already applied for or obtained a Graduate visa before the anticipated start date of employment. NHS Cambridgeshire and Peterborough based roles will prioritise applicants at risk of redundancy, though interviews cannot be guaranteed.


Qualifications

* Registered Nurse RSCN/RN Child Branch
* Degree in a related subject
* Level 3 safeguarding
* Evidence of continuous professional development
* Specialist Practitioner in Children’s Community Nursing
* Master’s in a related subject
* Advanced clinical assessment
* Advanced communication skills/counselling qualification


Experience

* Significant experience working with children and young people with complex needs in community settings
* Experience managing a patient caseload
* Experience teaching parents, colleagues, and students
* Experience initiating change
* Experience working with acutely unwell children in a hospital setting
* Experience with children with complex needs in a hospital setting
* Experience setting up care packages for children with complex needs
* NHS experience


Knowledge

* Understanding of care packages and commissioning structure
* Understanding of Clinical Governance
* Understanding of patient pathway complexities and how to support the patient and family


Skills

* Good verbal and written communication skills
* Capable of working as an integral member of a small team
* Adaptable to changing situations due to workload and colleagues’ needs
* Ability to work unsupervised and manage own workload
* Effective interpersonal skills
* Ability to lead developments in practice
* Commitment to multidisciplinary working
* Demonstrate analytical and judgemental skills
* Ability to quickly appraise situation and act accordingly
* Accept personal accountability for own practice
* Competent at managing tracheostomy’s victor


Additional Requirements

* Act in a professional manner at all times in accordance with the NMC Code of Conduct
* Capable of developing liaisons with other practice areas/academic institutions
* The ability to understand and behave at all times towards patients, visitors and colleagues according to the Trust values of safe, kind, excellent
* Knowledge of wider issues related to the provision of community care for children with complex needs
