Job Summary
The Bristol Dementia Wellbeing Service, a joint venture between Alzheimers Society and Devon Partnership Trust, provides comprehensive dementia care for people registered with a Bristol GP. This role is a Band 6 Senior Dementia Practitioner, working full‑time (37.5 hours per week) with a liaison model across care homes and GP surgeries.
Main Duties
* Deliver community‑based dementia support, including assessments, case management and coordination of personalised care plans.
* Provide specialist expertise to GPs, primary care teams, care home staff and other stakeholders, offering advice and training on dementia care.
* Lead clinical supervision and support for other team members, contributing to service development and evaluation.
* Maintain flexibility for extended hours at the Access Point (08.00‑18.00 M‑F) and travel across Bristol as required.
Qualifications
* Professional mental‑health registration (e.g., RMN, Social Work, OT, Community Nursing).
* Optional: Non‑medical prescriber or other recognised therapeutic qualification.
* Post‑graduate qualification relevant to dementia care a plus.
* Training to level 3 in safeguarding adults and children a plus.
Experience
* Knowledge of the Mental Health Act and community‑based care for older adults.
* Experience managing complex clinical situations and delivering multidisciplinary care.
* Understanding of the SAP process and diabetic care principles where applicable.
Skills & Abilities
* Excellent written and verbal communication, capable of engaging a diverse range of professionals and users.
* Strong organisational and planning skills, with the ability to work independently or as part of a team.
* Proficiency with IT systems including electronic record‑keeping.
* Capacity to travel across Bristol and manage time flexibly.
