This band 6Childrens Community Nurse post, based within a team in Redhill, is an exciting opportunity, where no two days are the same, to make a difference to children and young people requiring nursing care and support in their homes/nurseries/school.
You will be responsible for delivering a high standard of nursing care, have excellent communication skills andbe ableto use your own initiative as well as working well as a team player. You will be supported to develop your expertise both within your Team and by regular reflection and professional training.

Main duties of the job
Delivery of high-quality nursing care to children and young adults aged0-18 years in their homes, nurseries, schools in a designatedgeographical locality in Surrey. To work withoutdirect supervision and deputise for the clinical team leader in their absence.

Principalduties:
Practicewithin all the NMC Professional Codes and Standards and the organisationspolicies and procedures
Responsiblefor providing day-to-day support and supervision to Band 5 Community StaffNurses within team, acting as a role model who motivates
Responsiblefor deputising for the clinical team lead
Responsiblefor working at weekends on a rotational rota and support a 24-hour end of lifecare rota as required
Responsible,in conjunction with the Clinical Team Leader for the planning and organisationof the Childrens Community Nursing team including allocating and re-allocatingstaff according to patient need and the demands of the service and ensure theteam delivers responsive care within the allocated resources.
Assesses,determines and records complex treatment plans (including physiological and/orpsychological functioning) which are consistent with the outcomes of anassessment
Actsas an advocate to produce care plans in collaboration with other agencies andprofessionals that reflect childs/young persons/families wishes and views
Respectsindividual wishes, beliefs and consent to treatment ensuring privacy anddignity is maintained at all times
Maintaina high level of clinical skill care and use of central lines, enteralfeeding devices, catheterisation, management of long-term ventilation,tracheotomy care, wound care, stoma care, administration of specialistinfusions within the community setting and end of life care.
Work together with symptom managementteams to provide a holistic approach to care for children and families,anticipate their needs and provide comfort and support for the child and thefamily requiring palliative care.
Undertakephysical examination of the child/young person which may include listening tothe heart and lungs, undertaking venepuncture, accessing central lines/ports,catheterisation, change of tracheostomy breathing tubes, replacement ofPercutaneous Endoscopic Gastrostomy (PEG) feeding tubes etc.
Competentin the use of various equipment that requires accuracy within a narrow marginfor error syringe drivers, ventilators, saturation monitoring,enteral/nasogastric/nasojejunal feeding pumps etc.
Take a lead role in developing andimplementing the seamless transition of care, supporting early hospitaldischarge and continuity of care for children and their family.
Workwithin the delegated budget to ensure effective use of materialresources/supplies
Participatesin ordering equipment from centralised equipment store
Communicatingand receiving sensitive, complex and sometimes contentious information wherepersuasive, motivational, negotiating, empathetic or reassurance skills arerequired
Responsiblefor maintaining accurate and contemporaneous records at all times
Communicateseffectively with multi-disciplinary team and contributes to meetings withprofessionals, parents, children child protection case conference, networkmeetings, safeguarding processes, discharge planning etc.
Effectivelycommunicates information to all members of the team, including sharingresearching findings and necessary changes in practice and that issues areappropriately brought to the attention of the clinical team lead
Anticipatesand manages barriers to understanding language, interpreters, visual andhearing impairments
Establishand maintain a robust communication pathway with acute settings on all mattersthat affect the management and readmission of child into hospital
Managesdifficult situations to achieve resolution across a range of situationsincluding dealing with complaints and incidents to avoid escalation wheneverpossible.
Participatesin the management of emergency situations including major incidents
Responsiblefor review, analysis and comparison of clinical and staff data that informs theplanning and evaluation of the teams requirements
