A Practice Nurse plays a key role within a General Practice, delivering highquality, patientcentred nursing care as part of the primary care team. They work closely with GPs, Nurse Practitioners, Health Care Assistants, and wider multidisciplinary colleagues to support the health and wellbeing of the practice population

1. Chronic Disease Management
Provide structured reviews for long term conditions such as:
* Asthma
* COPD
* Diabetes
* Cardiovascular disease
* Develop personalised care plans and provide patient education.
* 2. Treatment Room Duties
* Wound assessment and management
* Dressings and suture removal
* ECG recording
* Urinalysis
* Blood pressure monitoring
* Contraceptive advice (nonprescribing)
* Childhood immunisations
* Seasonal flu and COVID19 vaccinations
* Adult vaccinations as per national schedules
* 4. Health Screening & Preventative Care
* Cervical cytology (if trained)NHS Health Checks
* Lifestyle advice: smoking cessation, weight management, alcohol advicE
* Health promotion within the practice population
* 5. Clinical Quality & Governance
* Maintain accurate, timely clinical records
* Contribute to QOF, audits, and quality improvement projects
* Adhere to infection prevention and control standards
* Support safeguarding processes for adults and children
* Work in line with practice policies, guidelines, and professional standards
* 6. Teamwork & Communication
* Work closely with GPs, the HCA, admin team, and wider PCN colleagues
* Participate in team meetings and contribute to practice development
* Provide support and mentorship to junior staff where appropriate

* Maintain NMC registration and revalidation

* Engage in annual appraisal and competency reviews

Qualifications

* Registered Nurse (NMC)
* Experience in primary care, community, or acute nursing
* Immunisation and vaccination training
* Chronic disease management knowledge
* Ability to work autonomously
* Strong communication and interpersonal skills
* Cervical cytology training
* Chronic disease qualifications (e.g., diabetes, respiratory)
* Experience using EMIS
* Mentor/teaching experience
* Interest in quality improvement or audits
