Typical day as a Reablement Worker
Working closely with the care team to follow support plans, reporting concerns.
Help customers regain or maintain independence to live well in their own home by carrying out every day daily living activities themselves.
Carrying our risk assessments, support planning, goal setting and regular reviews.
Supporting customers who have more complex needs with personal care including bathing, toileting and getting dressed.
Supporting our customers in their own apartment who have a range of support needs including frailty and dementia.

You bring
A level 2 or above, or equivalent in social care or being willing to work towards this
Care Certificate or willing to complete within the first 12 weeks of employment
If youve worked in a CQC care setting previously, that would be great!
Have a positive, caring can-do attitude, and empathy to really make a difference to our customers lives.
Previous experience of working with support plans, recording and using information concisely and effectively.
Knowledge of local community and networks would be brilliant.

Job details
We provide 24-hour support.
Night shifts with some lone working, plus regular team meetings and training.
Core shifts are Thursday, Friday and Saturday evenings.
Occasionally our customers' needs mean your start and finish times may change. Where we ask you to be flexible, we will always try to reciprocate
Confident using technology for plans, online learning and teamwork
Youll need an Enhanced DBS check done and we pay for that.
