Complex Care Case Manager (Clinical)\n£35,000 (negotiable) plus £1500 quarterly bonus\nManchester\nFull-time, 35 hours per week\nWe are seeking a motivated Complex Case Manager who is dedicated to positively impacting our clients and promoting a positive atmosphere within their care team. Join us in creating a caring community that values and uplifts both clients and staff members, and your role will be instrumental in this mission.\nResponsibilities:\n * Act as a role model, fostering a supportive environment to empower staff in delivering quality care.\n * Uphold internal and external standards, adhering to CQC regulations, contractual requirements, NICE guidelines, company best practices, and Equal Opportunities.\n * Take joint ownership of a caseload of complex clients and manage their care package holistically, in collaboration with their appointed Nurse Case Manager.\n * Develop, review, and implement client care plans and risk assessments following CQC regulations and best practice guidance.\n * Co-ordinate and lead the transition and ongoing continuity of all allocated complex care packages, working with clients, professionals, and Nurse Case Managers.\n * Maintain high standards of documentation and record keeping in compliance with CQC, national legislation, and local standards.\n * Conduct regular auditing of records and daily evaluation notes, reporting trends and taking appropriate actions.\n * Inform the Service Manager of any issues that may impact the quality of care or the health and safety of clients and healthcare workers.\nGeneral:\n * Comply with the Company's procedures regarding Conduct and Confidentiality.\n * Take responsibility for personal health and safety, as well as the health and safety of others.\n * Continuously develop professional skills through relevant training and updates.\n * Ensure systems and databases are kept up to date.\n * Complete any other tasks assigned by the Line Manager within the scope of the position.\nEmployee Benefits:\n * 25 days of paid holiday\n * 8 days of paid Bank Holiday\n * Extra day off for your birthday\n * Opt-in to work-based pension\n * Full support and guidance to perform job responsibilities effectively\n * Opportunities for career progression and continuous professional development (CPD)\n * Full induction provided\n * £1500 quarterly bonus\nFor more information or to apply for this Complex Care Case Manager role in Manchester please contact Chloe at Connect Care Recruitment.\nAlternatively, click APPLY now
