We are looking for an experienced and dedicated Social Worker to join our Hospital In-Reach Team. This role offers an opportunity to be part of a forward-thinking approach to admission avoidance, discharge planning, and recovery. Based at Great Western Hospital, you will work closely with healthcare partners adopting a collaborative approach to achieve the best outcomes for the people we serve.
The In-Reach social work role is developing, and we seek an innovative and passionate individual to help expand this role within the team. You will find a supportive environment with flexible working options, allowing personal and professional growth. This role requires working on a rota basis, 5 days over 7, including 1 weekend in 3.
Main duties of the job
As an experienced Social Worker, you will provide advice, assessments, and support for individuals moving from hospital to home or care settings. You will work closely with healthcare colleagues to ensure patients are on the correct discharge pathway from the outset. This is a fast-paced role requiring creativity, reflexivity, and autonomy to make timely decisions.
To succeed, you need a Social Work qualification, registration with Social Work England, and knowledge of safeguarding and health and social care legislation. Strong communication and IT skills are essential, along with a commitment to achieving the best outcomes for patients.

Job responsibilities
Specific duties include:
* Providing in-reach social care to support timely hospital discharges, including working weekends on a rota basis.
* Collaborating with acute health partners, including Health in-reach services and the ASC Flow & Triage Advanced Practitioner.
* Offering proactive social care support at the hospital front door to prevent admissions and reduce length of stay.
* Identifying and addressing barriers to discharge, undertaking assessments, and sharing information with MDT colleagues.
* Signposting customers to appropriate support services and supporting the Care Transfer Hub function.
* Representing ASC on Care Transfer Hub calls during weekends and making joint discharge decisions with the MDT.
* Providing on-call in-reach support at weekends (1 in 3 weekends).
* Conducting person-centred assessments, including Mental Capacity Assessments, and planning support to promote independence.
* Ensuring accurate recording of assessments and activities in line with targets and policies.
* Gathering sufficient information to assess customer needs and signposting to local services, including voluntary and community sectors.
* Managing risks and safeguarding, including acting as Investigating Officer when necessary.
* Working within a multidisciplinary team to ensure effective communication and decision-making.
* Adhering to council policies and contributing to team development.
* Engaging in supervision and professional development, representing the council positively at all times.
