We are looking for a Physiotherapist to join the North Lancashire Community Therapy Team, based from the community offices at Lancaster.
The service works closely with the Integrated Care Communities, local GP's, Adult Social Care, Advanced Clinical Practitioners and hospital discharge teams. The service supports people with a wide variety of conditions including elderly care, frailty and falls, orthopaedics and long-term condition management.
Responsibilities
* work as an individual within the community therapy team undertaking specialist assessment, diagnosis and treatment of patients with frequently complex problems in their own homes, residential and nursing care homes or our 23-bed residential rehabilitation unit.
* have completed a range of core rotations within the NHS as a physiotherapist with a desire to develop your clinical skills further
* demonstrate and evidence your own professional development
* demonstrate your ability to work independently when necessary
* have a friendly and flexible attitude, which is patient focused and dedicated to quality care
* use your expertise to supervise and teach physiotherapy students and other staff within the service
We operate from three main hospitals-Furness General Hospital (FGH) in Barrow, the Royal Lancaster Infirmary (RLI), and Westmorland General Hospital (WGH) in Kendal, as well as a number of community healthcare premises including Millom Hospital and GP Practice, Queen Victoria Hospital in Morecambe, and Ulverston Community Health Centre.
FGH and the RLI have a range of General Hospital services, with full Emergency Departments, Critical/Coronary Care units and various Consultant-led services.
WGH provides a range of General Hospital services, together with an Urgent Treatment Centre, that can help with a range of non-life threatening conditions such as broken bones and minor illnesses.
All three main hospitals provide a range of planned care including outpatients, diagnostics, therapies, day case and inpatient surgery. In addition, a range of local outreach services and diagnostics are provided from community facilities across Morecambe Bay.
