Care Coordinator- Telford

We're now looking for a Care Coordinator in Telford to help us deliver outstanding care and keep our service running smoothly. This role is central to making sure our clients get consistent, safe, and compassionate support every day.
Key Responsibilities:
* Call Management and Rota Coordination
* Manage incoming calls from Care Assistants in your area
* Oversee rotas, reallocating holiday, sickness, and emergency cover quickly and effectively
* Maintain continuity of care for service users during rota changes
* Deliver care in the community when needed to support the team
* Escalation and Crisis Management
* Escalate issues to families, management, or directors where appropriate
* Act as the first point of contact for Senior Care Workers and Care Workers out-of-hours
* Make safe, timely care decisions during incidentsEmergency and On-Call Support
* Provide cover for colleagues during emergencies to ensure no disruption in care
* Take part in the out-of-hours on-call rota, providing emergency support in person if necessaryDocumentation and Communication
* Record all incoming calls in daily communication logs and provide clear handovers
* Document rota changes, concerns, and extra calls for Finance accurately
* Attend review meetings, keeping detailed notes and updating the system with follow-up actions
* Liaison and Reporting
* Liaise with social workers, hospital staff, GPs, and service users' families
* Record and report concerns to the appropriate authorities with accurate documentation
* Customer Care and Company Image
* Ensure an exceptional level of customer care at all times
* Represent Edan Support positively with service users, families, and professionals
* Keep up to date with current regulations and company policies
* Team Support and Additional Duties
* Plan and manage rotas using Access CarePlanner and PeoplePlanner
* Support recruitment, training, and staff management as required
* Undertake any additional tasks assigned by management to keep services running smoothly
Essential Skills and Experience:
* Minimum 2 years' experience delivering hands-on care in domiciliary or community settings
* Proven rota management experience with Access CarePlanner and PeoplePlanner
* Must hold a full UK driving licence and access to a vehicle
* Strong organisational skills and ability to handle fast-paced situations calmly
* Excellent communication skills, both written and verbal
* Proficiency with Microsoft Word, Excel, and other IT systems
* NVQ Level 2 in Health and Social Care (or equivalent), or working towards it
* Solid knowledge of CQC standards and regulations
* Successful applicants will be subject to three good references and an enhanced DBS check.
