We are currently looking to recruit a permanent, Senior Practitioner Social Worker, to join the Integrated Triage and Assessment team, Porth Gofal Service.
This post is an exciting opportunity for a dynamic and committed member of staff to work in a multi-disciplinary environment which will support vulnerable individuals and their families within a hospital and community settings.
Porth Gofal is the integrated decision making hub for Ceredigion, that triages and determines the level of need of all cases and the best outcomes for those individuals.
Porth Gofal is the heart of service provision that focuses on ensuring that Citizens receive the intervention that is best placed to meet their needs, or where else, to guide them to early help or specialist services.
What Porth Gofal provides:
1. a responsive service to Citizens living in Ceredigion
2. applies a Strength based needs assessment focusing on service user choice and control using the Signs of Safety framework
3. a reduction in the time Social Care professionals spend in determining eligibility for statutory services
4. legislatively compliant service that meets the statutory requirements of the SSWBA
5. a safe service that identifies vulnerable and at risk Citizens, that responds effectively
What Porth Gofal achieves:
6. provides information, advice and assistance about community health and social care services for adults that improves the connection between people and their communities
7. encourages a balanced approach to the assessment of need ensuring that citizens are at the centre of conversations, decisions and the “what matters” conversation takes place
8. multidisciplinary triaging of referrals to identify the right professional to respond e.g. physiotherapist, Occupational Therapist or Social care workers to ensure right response at the right time
9. provide more consistent, coordinated assessment for care and support
10. improve the flow of information between services, resulting in faster decisions being made about people’s needs and support
11. reduces hospital admissions by providing rapid wrap around reablement response service
12. provides an in-reach service to reduce delayed transfers of care by providing timely discharges from hospital
A commitment to integrated working with a focus on positive rehabilitation outcomes for the citizens is essential.
The ability to travel throughout the county is essential.
Must have two years post qualifying experience with experience of supervising staff.
A bilingual policy is in operation and candidates who do not have the linguistic ability in Welsh will be supported and encouraged to achieve the ALTE standard within two years.
