NHS Greater Glasgow and Clyde (NHSGGC) is one of the largest healthcare systems in the United Kingdom, employing approximately 40,000 staff across a wide range of clinical and non-clinical professions and roles. We deliver acute hospital, primary, community and mental health services to a population of over 1.15 million people, and to a wider population of 2.2 million when regional and national services are included.

The role:

As an integral member of the Inverclyde Older Persons Mental Health Team, the post-holder will work in partnership with team colleagues and the wider HSCP. The post holder must demonstrate compassionate caring behaviours which support the key team functions including Memory Service, Community Mental Health Nursing and Physical Healthcare Clinic. The post-holder will provide care and support as part of an agreed plan of care which involves the use of strength based recovery focused approach that proactively involves the person and carers. As an integrated health and social work team, the postholder will also be required to provide support, when necessary, to social work colleagues.

Duration, Location, and Working Pattern:

1. Based in Crown House
2. Full-time position (37 hours per week)
3. Permanent Contract
4. Shift pattern for this post are 9am-5pm Monday - Thursday & 9am-4.30pm - Friday

Key responsibilities:

5. Functioning as support worker within a multi-disciplinary team, to a defined patient group

6. Contributing to the nursing assessment by implementing and reporting on aspects of the prescribed plan of care to the named nurse/ keyworker

7. Contributing to risk assessment and risk management planning by identifying and reporting issues of concern to the named nurse/ keyworker and supporting multi-disciplinary team compliance with NHSGGC policy and procedures

8. Carrying out all relevant forms of nursing care pertinent to the individual needs of people within the defined patient group

9. Identifying and responding to the health promotion needs of patients focusing on improving the mental well-being of patients

10. Identifying and responding to the health promotion needs of patients focusing on improving the physical health of patients

11. Identifying and responding to the needs of patients which might be related to vulnerability, gender health, domestic abuse, child protection and/or self harm.

12. Maintaining a good standard of record keeping that meets the standard expected of the inpatient service.

13. Raising and escalating any cause for concern through established reporting mechanisms that may significantly impede the delivery of safe, effective person centred care.

Knowledge, training, qualifications and/or experience required to do the job:

Essential

14. Driving licence
15. Working knowledge of mental health nursing care
16. Working knowledge of mental health issues, particularly organic illness
17. Evidence of using digital technology, information systems, email and basic applications
18. Evidence of skills and behaviours that demonstrate effective written and verbal communication
19. Evidence of skills and behaviours that demonstrate application of HCSW Standards and Code of Conduct
20. Ability to form working relationships with others
21. Ability to follow instructions and undertake work without direct supervision

Desirable

22. Educated to or actively working towards SVQ3/SCQF Level 6 or equivalent experience
23. Completion of a health/mental health/social care programme or equivalent experience
24. Knowledge of health and social care issues that impact mental health and wellbeing
25. Experience of using cognitive screening tools such as ACE-III/MMSE
26. Trained in venepuncture
