Overview
We are looking for experienced qualified social workers to join the Integrated Learning Disabilities Assessment & Care Management Team. Our team is part of an integrated health and social care service providing a wide range of support to people with learning disabilities and their carers. The team consists of community nurses, psychologists, occupational therapists, physiotherapists, speech and language therapists, psychiatrists and an employment service, enabling a holistic approach to support.
Responsibilities
* Provide social work assessment and care management for people with learning disabilities.
* Develop and implement care plans in conjunction with service users.
* Collaborate with multidisciplinary team members to deliver comprehensive support.
* Advocate for user independence, self‑determination and wellbeing.
* Promote the use of direct payments to maximise autonomy and flexibility for users.
* Perform frontline duty and manage a complex caseload.
Qualifications
* Have the skills to work with vulnerable service users and their carers.
* Be committed to the principles of personalisation, strengths‑based approach and supporting people to maximise their independence.
* Be creative in developing support plans in conjunction with service users that demonstrates best value and community inclusion.
* Have knowledge of safeguarding adults’ practice and legislation.
* Possess the skills to hold a complex caseload and work on frontline duty.
