The role will include tasks such as:
* Helping people to leave hospital promptly and safely transition back to independent living at home.
* Visiting Service Users in their own homes to provide care and support as outlined in their Care Plans.
* Providing personal care, administering medication, monitoring vital signs, assisting with exercises, and domestic tasks if needed.
* Liaising with medical professionals such as district nurses and pharmacies.
The aim is to rehabilitate and maximize potential, supporting Service Users in activities that promote independence and monitor well-being.
Working hours are between 7am-10pm with shift patterns of 7am-2pm and 4pm-10pm, including alternate weekends.
We offer competitive pay rates of £13.43 per hour on weekdays and £15.50 per hour on weekends.
*Note: The hourly rate is subject to holding a NVQ Level 2 or above in Health and Social Care.
