This post is within the Winsford Care Community. The role is to assess, manage and provide treatment to a caseload of service users with complex and/or multiple needs. They will be working closely with the MDT and provide supervision to therapy assistants, junior staff and students. There may be an expectation to work between multiple sites in the organisation.
This is a permanent post. The Occupational Therapist will be expected to join the 7-day, urgent community response rota and may be lone working within the guidelines of the lone working policy. Previous applicants within the past 3 months need not apply.
We are looking for enthusiastic, motivated and flexible Occupational Therapist to join our friendly, supportive, dynamic team in Winsford Care Community. We provide a rapid, unplanned and planned response, facilitate early discharge from hospital and aim to reduce hospital admissions. We also cover Elmhurst.
There is a varied caseload and you will need to demonstrate knowledge, experience and relevant clinical skills with patients with complex / long term conditions, falls, orthopaedics, general mobility problems and neurological conditions.
The successful candidate will work as a member of our integrated multidisciplinary team alongside District Nurses, Advance Nurse Practitioners, Complex Case Practitioners, MacMillan Nurses and will be responsible for supervising junior members of staff and students.
There will be associated administrative duties and you will need to be computer literate with a good knowledge of Microsoft packages and have accurate keyboard skills. Our team utilises digital technology to support remote working and accommodate a flexible working approach where possible and is supported to do this with access to a laptop and mobile phone.
You are required to be a car driver and have access to your own vehicle.

Clinical Practice
* Manage a clinical caseload with autonomy, work without direct supervision
* Undertake comprehensive physiotherapy assessment, provide a therapeutic diagnosis, and implement agreed treatment plans for complex service users with multiple pathologies, ensuring privacy, dignity and individuality
* Provide individual or group therapy as appropriate to setting
* Implement monitoring and evaluation of treatment to measure progress and outcomes
* Develop programmes of care to support discharge from service
* Be responsible for equipment used in carrying out clinical duties, and to adhere to policy, including competency to use equipment and to ensure safe use of equipment by others through teaching, training and supervision
* Record, maintain and store accurate documentation; communicate outcomes of assessments/interventions to appropriate disciplines
* Arrange appointments, work closely with MDT, attend case reviews/conferences/MDT meetings
* Contribute to safeguarding of service users/families as appropriate
* Assess capacity, gain valid informed consent and work within a legal framework, including with those lacking capacity, within organisational policies
* Be professional and legally responsible and accountable for all aspects of your own work
* Maintain Professional Registration with HCPC
* Work within organisational, HCPC and CSP guidelines; monitor quality of practice against national/local standards

* Participate in professional development programmes and provide advice, teaching and instruction to relatives, carers and professionals
* Help manage workload allocation within the specialist area
* Contribute to policy/service development within the team and lead delegated projects
* Undertake other duties as required by senior staff and deputise when needed
* Be aware of the financial impact of clinical decisions; support stock management
Education
* Organise and lead in-service training for Band 5 Therapists and Assistants under guidance of Band 7 staff
* Critically evaluate research and apply to practice; understand national guidelines and legislation affecting health and social care
* Train, supervise and support less experienced therapy staff, therapy assistants and students; conduct formal staff appraisals if required
* Maintain competency to practice through ongoing development and a professional portfolio
* Demonstrate specialist skills to maintain fitness to practice as a Physiotherapist
* Work within organisational policies and HCPC Code of Conduct; keep up to date with mandatory training
* Participate in evaluation of clinical practice and disseminate findings
Research, Audit and Governance
* Measure and evaluate work using evidence-based practice, audits and outcome measures
* Represent at local meetings and attend national meetings; recommend and implement changes under supervision
* Undertake audits and contribute to regional/national levels as appropriate
* Support research activity within the service
* Disseminate audit and evaluation findings to relevant areas
