Specialist Community Learning Disability Practitioner

An exciting opportunity has arisen for an enthusiastic, highly motivated, dynamic, experienced, and dedicated learning disability nurse to join our established innovative, West Cheshire Community LDmultidisciplinary team.

You will be joining a well-established and supportive team, where person centred care is at the forefront of everything that we do.

You will be responsible for delivering high-quality clinical care and supporting the clinical leadership within the community nursing team and wider CLDT, ensuring all work is needs led, evidence based and in line with current legislation.

You will also support the leadership and development of services in line with the workstreams that are the focus for the care group, which includes, mental health, forensic, physical health, neurodevelopmental conditions and cognitive impairment.

CWP hosts CANDDID, the Centre for Autism Neurodevelopmental disorders and Intellectual Disabilities, which carries out research, education and training, and all clinicians are encouraged and supported to develop research and QI skills.

There are two bases that the team works from, one in Chester (Eastway) and one in Winsford (Wyvern House), the location of base can be reviewed with the successful candidate.


Main duties of the job

The post holder will carry a caseload of clients with learning disabilities who have the most complex needs, where a very high level of clinical expertise is required to deliver care that will reduce health inequalities for people including behaviours of concern and/or autism.

The post holder will act as transformational role model/guide to deliver specialist learning disability assessment, intervention and person centred outcomes.

To lead on and support the development of effective services in terms of Positive Behaviour Support (PBS) and what this should entail and how best to deliver this, within the team and wider LD service.

To support the Intensive Support Function (ISF) within the team and wider LD service, as and when required, especially during periods of absence.

They will also be responsible for providing clinical supervision and appraisals of nurses within the team and supervise students while on placement with the team.


Person Specification


Qualifications

* Registered Learning Disability Nurse (RNLD; RNMH)
* Post registration degree level training e.g. Specialist practitioner or other equivalent degree (e.g. forensic behavioural science) or evidence of previously working at this level for a minimum of 12mths
* Evidence of post registration modules at degree / masters level e.g. - mentorship module - forensic module - CBT - challenging behaviour - autism - Clinical audit & research
* Nationally recognised Qualifications in challenging behaviour and / or autism (e.g. IABA; etc.)
* Nurse prescribing


Knowledge

* Ability to assess/ draw up clinical formulation
* Plan/Evaluate care to clients with complex needs
* Ability to take lead health coordination function for people with complex health needs
* Ability to undertake complex risk assessments and manage risks through coordinated multi agency approach to care


Experience

* Some experience of working with people with a learning disability in a community setting
* Direct experience of working with clients with complex needs i.e. challenging behaviour, autism, mental health problems
* Positive attitude to people with L.D and experience of person centred planning
* Experience in the development and implementation of complex care packages
* Experience of CHC process
* Staff Management experience (PDP's; sickness / absence management / recruitment& retention etc.)
