Fixed‑term (4 months) HIU Case Worker role providing front‑line, day‑time support to High Intensity Users in Northwick Park Hospital ED. This role liaises with the Borough HIU Care Coordinator, coordinates patient care plans, attends patients, and works to improve health outcomes for this cohort.
Key Responsibilities
* Accept referrals of HIUs attending ED who do not require acute medical attention within ED.
* Coordinate patient care plans and lead local HIU meetings.
* Maintain effective communication with key stakeholders to ensure care plans progress.
* Attend patients and assess immediate needs, including self‑harm risk and social issues such as housing, debt, or isolation.
* Review medical and social care records and existing care plans to formulate an immediate care plan, including referrals to suitable agencies.
* Co‑ordinate with statutory and non‑statutory services to enable access to local services, referring ongoing needs to the Borough Care Coordinator.
* Participate in Borough and local Trust HIU MDT meetings.
* Support the creation and maintenance of Urgent Care Plans in Cerner.
* Support and deliver training to ED staff on caring for HIUs.
All positions working with vulnerable groups will be subject to an enhanced criminal records check upon appointment, and staff will need to give consent for the Trust to receive regular updates.
