We have an exciting opportunity for a Social Care Worker to join our established Older Peoples Mental Health Team. The team provides personalised services to achieve better outcomes for older people and their families.
The work is varied, demanding and very rewarding. You will manage a varied caseload of people living at home, in residential /nursing placements and in hospital.
As a Social Care Worker, you will work closely with colleagues within the OPMH Team as well as partners from Health. You will receive regular supervision, access to training and development and the opportunity to make a difference.
