Key Tasks and Responsibilities of the Post:
Communication: Handle telephone enquiries from GPs, SWAST, health professionals, University Hospital Plymouth, patients, relatives, and external agencies. Update dashboards, spreadsheets, and data collection tools. Send communications and share information with the team or relevant recipients. Complete administrative tasks such as creating spreadsheets, minute-taking, taking messages, and utilizing IT programs.
Participate in virtual conferencing. Complete and send referrals for planned resources as appropriate. Ensure all resources are available for managers to triage and responders to visit. Provide quality advice and information to customers and professionals within your limitations.
Customer Plan Implementation: Complete administrative activities related to the customer plan, including documenting contacts. Ensure all components of the plan are implemented as requested, monitor actions, and ensure responses are delivered. Raise issues affecting plan implementation to the Responder or Team Leader. Action alternative options discussed with the Responder or Team Leader.
Communication with Stakeholders: Communicate relevant information daily to services and agencies such as GPs, SWAST, nurses, care agencies, customers, families, voluntary agencies, etc. Use appropriate communication channels (telephone, email, face-to-face) ensuring confidentiality, professionalism, and appropriateness. Ensure communication complies with policies and procedures.
Organizational Responsibilities: Manage and prioritize workload, meet response deadlines, and support an unpredictable work environment. Maintain accurate and up-to-date documentation on systems like Eclipse, Care First 6, SystmOne, following policies and legislation. Report system faults and queries. Collect and enter statistical data in line with policies.
Adhere to organizational policies including health and safety. Record accidents, incidents, or untoward events promptly. Stay updated on policy changes and implement relevant protocols and guidelines.
