Are you a passionate, innovative Occupational Therapist looking for a new and exciting challenge with older adults? An opportunity has arisen for a band 5 Occupational Therapist to join our newly formed community/inpatients OT team based in Tameside working with older people with mental health difficulties. Our patients come to us from many different settings needing assessment and support to remain as independent as possible. Patients can have a variety of mental and physical health conditions. The Occupational Therapy role is crucial in ensuring patients health, wellbeing and independence are appropriately assessed and facilitated. A part of the role is to support with discharge planning, working closely with the discharge social workers, including assessments on the ward and in the home setting. You will be based within a supportive team of OT’s and will have the opportunity to develop a range of skills around both functional and organic mental health conditions. There will lots of opportunities to work alongside the wider MDT consisting of consultant psychiatrists, nurses, CPN’s, community support workers/HCA’s, physiotherapists, psychologists, OT students and apprentices. This post would suit a passionate OT who is looking to develop their skills and practice in an Older Adult Mental Health setting.

This is a full-time post 37.5 hrs.


Main duties of the job

The postholder will provide occupational therapy to inpatients using evidence based, person centred principles as part of the multidisciplinary ward team. You will be responsible for delivering the occupational therapy pathway to service users on the ward, including assessment, facilitating interventions (individual and group) and reviewing and evaluating the outcome of agreed goals.

Main Duties and Responsibilities
Clinical
To use principles of evidence based Occupational Therapy to assess, plan, implement and evaluate interventions.

To prioritise service users’ occupational needs following screening, applying effective professional reasoning in line with the occupational therapy pathway.

To select and use validated assessment tools routinely, informed by the Model of Human Occupation, to identify service users’ occupational participation and performance needs.

To agree goals with the service user to address identified occupational needs.

To plan and implement individual and/or group interventions with the service user to achieve their identified goals

To undertake home and environmental visits and any necessary follow up visits as part of discharge planning.

To assess for, recommend and/or order equipment and adaptations necessary for safe community living.

To monitor and evaluate treatment to measure service users’ progress, prepare for discharge and ensure effectiveness of intervention

Detailed job description and main responsibilities

Communication and documentation

To establish effective communication networks with service users and carers, team members and other involved agencies for example, social services

To participate in multi-disciplinary meetings contributing to decisions regarding service users’ care

To provide support and education to service users and carers for example, fitting and use of equipment

To provide written and verbal reports to members of the multi-disciplinary team and involved others

To ensure that up to date written and electronic records and activity data are maintained in accordance with Trust policy.
Service Development and Delivery

To respect the individuality, values, cultural and religious diversity of service users and contribute to the provision of a service sensitive to these needs

To participate in the development and delivery of Occupational Therapy

To participate in the planning, evaluation and audit of practice, clinical pathways, policies, procedures and protocols within your area

To contribute to the service and team quality, safety and governance agenda

To demonstrate an understanding of national guidelines and legislation relating to health and social care and their impact on service provision

To be responsible for the effective use of resources needed to undertake service user interventions and treatment in accordance with professional and Trust standards.

Professional development

To comply with the Royal College of Occupational Therapists Professional Standards for occupational therapy practice, conduct and ethics and Health and Care Professions Council Standards of Proficiency for Occupational Therapists and national and Trust procedures.

To be aware of and work within the parameters of own knowledge, skills and personal limitations and seek appropriate advice

To actively participate in management and clinical supervision and appraisal

To review and reflect on own practice and performance through the effective use of management and clinical supervision and annual appraisal

To attend all essential to role training

To apply acquired skills and knowledge of professional practice in order to develop and maintain competence to practice as an Occupational Therapist

To maintain a professional portfolio for CPD recording learning outcomes through participation in internal and external development opportunities

To demonstrate the ability to critically evaluate current research and apply to own clinical practice

To participate in the induction, training and education of occupational therapy learners, other students and staff in the work area.

Education / Qualifications


Essential criteria

* Diploma/Degree in Occupational Therapy
* Registration with Health and Care Professions Council (HCPC)


Desirable criteria

* Membership of Royal College of Occupational Therapists


Experience


Essential criteria

* Range of clinical experience working in health or social care settings either on placement or within employment
* Experience of working within a multi-disciplinary team
* Experience of planning and facilitating 1:1 and group interventions


Desirable criteria

* Experience of working in mental health services
* Experience of working with partnership agencies including statutory and third sector
* Experience of contributing to the training and education of students and staff.


Knowledge


Essential criteria

* Knowledge of Model of Human Occupation and basic understanding of its application within mental health settings
* Knowledge of occupational therapy process (OT assessment, treatment planning, delivery and evaluation) and its application in mental health.
* Understanding of the nature of mental health conditions and how they can impact on service user functioning and recovery.


Desirable criteria

* Basic knowledge of mental health act, other relevant legislation and current mental health issues
* Ability to demonstrate a knowledge of risk and risk management.


Skills and Abilities


Essential criteria

* Effective written and verbal communication skills.
* Ability to work as an effective team member
* Ability to manage own time, to complete tasks and manage own caseload.
* Ability to report back service user outcomes and progress at professional/MDT meetings.
* Computer literacy
* Ability to value diversity and respond constructively to discriminatory behaviour


Work Related Circumstances


Essential criteria

* Use of a car or access to a means of mobility to travel across the Trust footprint in line with service needs
