Overview
You'll be at the heart of patient care, delivering evidence-based treatment for chronic conditions and promoting preventative healthcare. This role requires clinical expertise, innovation, and flexibility. You'll work within a multidisciplinary team across one of the client's group surgeries to improve patient outcomes and streamline services.
Responsibilities
* Providing patient-centred care for chronic disease management
* Supporting health promotion and prevention initiatives
* Participating in clinical audits, protocol development, and quality improvement
* Working resourcefully as part of a team in a fast-paced, agile environment
* Using critical thinking to make sound clinical decisions
Requirements
* Qualified and compassionate General Practice Nurse with experience in primary care
* NMC registration with current UK professional registration
* Minimum 1 year of experience in General Practice
* Minimum 2 years post-registration experience
* At least 1 year of recent community nursing experience
* Experience managing long-term conditions
* Background in clinical governance and quality initiatives
Essential Skills & Knowledge
* Cervical cytology, immunisations, ear care, minor surgery
* Change management and lifestyle support
* Excellent communication and IT skills
* Familiarity with public health policies and local health strategies
* Clinical supervision and mentorship experience
