The Leeds Social Prescribing service aims to improve physical, mental, and social health and wellbeing through a holistic approach to enhance quality of life, confidence in self-management, and supporting independence. In this role, you will need to establish and maintain strong working relationships with other providers, organizations, and local authorities to promote the benefits of social prescribing.
