Job Description
Liquid Personnel is recruiting Social Worker for it client's Community Mental Health Team (CMHT) located at Rochdale.
Responsibilities
* Undertake Strengths-Based Needs Assessments and review care packages for adults with severe and enduring mental health needs
* Update care plans and complete Mental Capacity Assessments where required
* Carry out safeguarding work in line with statutory duties when appropriate
* Support timely hospital discharges for individuals under secondary mental health services
* Work with both in-borough and out-of-borough placements; understanding of mental health pathways is beneficial
* Great rate of pay £30 per hour

Qualifications & Experience
* Social Work England registration
* Eligible to work in the UK
* Hold a full UK driving licence
* Post-qualified experience
* Degree level or equivalent in Social Work
