The Community Mental Health Nurse is a key clinical leader within the multidisciplinary care team, responsible for assessing, planning, coordinating, delivering, and evaluating care. The role involves communicating findings, influencing change, and promoting health, best practices, patient safety, and experience among our caseload of older adults.
The post holder must hold a full, valid UK driving license and have access to a car/vehicle for business purposes (unless disabled as defined by the Equality Act 2010).
Responsibilities include:
1. Carrying out comprehensive, systematic nursing assessments considering mental, physical, social, cultural, psychological, spiritual, and environmental factors in partnership with service users and others.
2. Collaborating with other disciplines, service users, and carers to develop holistic, person-centered care plans addressing identified needs.
3. Delivering and evaluating safe, person-centered care to support recovery.
4. Supporting best practice in line with NICE guidance as a clinical leader.
5. Facilitating and reviewing quality improvement initiatives within the care setting.
