Job Summary

The Specialised Care Home and Enhanced Dementia Service provides psychosocial interventions at times of crisis as alternatives to admission to our later life dementia wards. The team works in two parts: supporting individuals and carers in their own homes, and supporting those who reside in care homes.

We sit alongside the Later Life Community Mental Health team, sharing a pool of experienced later life medics. The team is made up of nurses, a social worker, occupational therapists and support workers.

Main duties of the role

The Specialised Care Home and Enhanced Dementia Service is a specialist team aimed at providing a safe and supportive alternative to a psychiatric hospital admission for adults with a diagnosis of dementia. We work with individuals, families and carers who are experiencing increasing difficulties due to behavioural and psychological symptoms of dementia. We also support individuals who have a long standing functional mental health illness, which may be complicated by their dementia diagnosis.

The team provides a range of psychosocial interventions utilising key models and formulations, non‑pharmacological approaches/interventions, and when indicated we provide pharmacological interventions.

Key responsibilities include:

* Conducting robust assessments including risk assessment, identifying needs and formulating care plans to meet identified needs/mitigate risk.
* Planning, delivering and reviewing effective "wrap around" interventions to manage risk and prevent hospital admission.
* Working with other services to provide comprehensive care and support.
* Performing assessments using standardised tools, considering culture, functional needs, carer needs, risk, physical health, safeguarding and mental capacity.
* Developing and delivering complex treatment programmes using evidence-based practice frameworks.
* Delivering individual or group therapeutic interventions, psychosocial interventions, motivational and coping strategies, medication management and interventions under the Mental Health Act.
* Providing support to carers, including developing crisis plans, rapid access plans and advance statements.
* Maintaining accurate health and social care records (paper and electronic) and reporting within agreed timelines.
* Leading and mentoring colleagues, providing clinical leadership and supervision, and participating in management and clinical supervision.
* Ensuring protection of individuals from abuse and harm and coordinating protective interventions.
* Maintaining health, safety and security, and contributing to professional development and team working.
* Managing own and team workload effectively using electronic resources.
* Adhering to professional codes of conduct and maintaining required skills and competencies.

Person Specification

Essential

* Education and registration: Diploma level/Degree in relevant health/social care profession (e.g., RN, OT, Social Worker).
* Current relevant registration with the NMC or HCPC and commitment to CPD.

Desirable

* Demonstrated post‑registration experience in frontline settings.
* Ability to deliver key therapeutic interventions such as CBT and psychosocial approaches.
* Understanding of recovery principles and secondary mental health services.
* Experience of undertaking carers’ assessments.
* Experience of mentoring/assessing students and learners.
* Experience with service users with dementia, older adults and/or crisis services.
