The Neighbourhood Night Service operates from 20:45 07:15, seven days per week. We strongly recommend getting in touch to discuss the role further.
Main duties of the job
Youwill be a registered staff nurse and have excellent assessment, planning andevaluation skills to deliver clinical care to patients and support our NursingAssistants overnight.
You will clinically coordinate and manage care ensuring that all calls areeffectively assessed, triaged, allocated and recorded. You will have anoverview of the service as a whole to ensure that patients and staff remainsafe and care is appropriate and effective overnight. re-allocating workto cover unexpected staff absence.
You must have excellent communication, organisational and IT skills as you willbe coordinating care for a large number of complex patients.
You will provide clinical leadership, including line management of Band 5 StaffNurses, and oversee line management of our Band 3 Nursing Assistants.
You must be able to think on your feet, problem solve and juggle conflictingpriorities as well as motivate others in challenging times and utilise theresources available to you effectively.

Job responsibilities
The Community Neighbourhood Nursing Night Service operates over a 24hour period, providing holistic high quality domiciliary nursing care topatients registered with a Leeds General Practitioner. The service delivers care overnight as partof the Health and Social Care Team within each Neighbourhood.
The service aims to reduce hospitalisation and promote dischargesthrough robust multi-disciplinary working with internal services, externalservices / organisations and the third sector.The service supports palliative care patients to die in their preferredplace of care.
The key workingrelationships include the Neighbourhood TeamsClinical Leads, Joint Care Management, external agency providers, MarieCurie, Commissioners and AdministrativeStaff.
