Job Summary
Following the retirement of one of our long standing GPs, we are seeking a new GP at our innovative GP practice now managed by the GP federation. We champion the doctor patient relationship & continuity of care at practice level, underpinned by the expertise of the federation & at scale services.
The workplan we are seeking to fulfil is for the following 6 sessions: Monday – Duty Day; Tuesday and Wednesday – Routine Days (hours can be flexible to allow for approx. 8 hours per day). There is also some flexibility for an additional 7th routine session. We would consider this being undertaken by 1 or more GPs. We take as much admin burden away from you as possible, support you with other members of the MDT giving you more time with each patient. Develop a clinical interest. 15 min booked F2F appointments. No more than 16 booked patient contacts per session. Reducing clinical admin, referrals, results, letters due to our centralized functions. We will also consider the GP Retainer Scheme.

Job Responsibilities
* Clinical Responsibilities: In accordance with the practice timetable, undertake a variety of duties including surgery consultations, telephone consultations and queries, visiting patients at home, checking and signing repeat prescriptions and dealing with queries, paperwork and correspondence in a timely fashion
* Make professional, autonomous decisions in relation to presenting problems
* Assess health care needs of patients with undifferentiated and undiagnosed problems
* Screening patients for disease risk factors and early signs of illness
* Developing care plans for health in consultation with patients and in line with current practice disease management protocols
* Providing counselling and health education
* Admitting or discharging patients to and from the caseload and referring to other care providers as appropriate
* Recording clear and contemporaneous consultation notes to agreed standards
* Collecting data for audit purposes
* Compiling and issuing computer-generated acute and repeat prescriptions
* Prescribing in accordance with the practice prescribing formulary (or generically) when clinically appropriate
* Undertake all the normal duties and responsibilities associated with a GP working within primary care

Qualifications
Essential
* Basic medical degree and completion of GP vocational training or equivalent (JCPTGP) certificate
* MRCGP
Skills & Knowledge

* Excellent clinical, communication and interpersonal skills
* Ability to work well within a team
* IT literate
* Demonstrable evidence of clinical governance activities
* Knowledge of EMIS Web and other IT systems
Desirable
* In-depth knowledge of the Quality and Outcomes Framework (QOF)
