The Quest Matron will be part of a multi‑disciplinary team responsible for supporting and advising staff in care homes on the prevention and proactive management of the needs of older people. The key aims of the service are to reduce urgent admissions and attendances to hospital from care homes, reduce unnecessary reactive demand on GPs, and provide a seamless, high‑quality, integrated approach to caring for people in care homes.
Responsibilities
* Minimise the effect of long‑term conditions on patients’ lives by providing crisis intervention, stabilising conditions, and preventing unnecessary A&E attendances, hospital admissions, and decreasing length of stay.
* Assess individual patient needs for those identified as high risk of admission to improve quality of life by identifying, coordinating, and linking to appropriate care services and therapy throughout the care pathway.
* Work with specialist teams and primary care colleagues to develop and implement a NICE guidance‑compliant service that supports multidisciplinary team working.
* Develop patient‑centred goals and encourage self‑management and self‑care approaches.
* Support the development of innovative, evidence‑based practice and adopt a multi‑agency approach to delivering care to care home residents.
* Reduce unnecessary reactive demand on GPs.
* Provide a seamless, high‑quality, integrated approach to caring for people in care homes.
* Enhance the quality of life for people with long‑term conditions.
* Help people recover from episodes of ill‑health or injury.
* Ensure people have a positive experience of care.
