In this role, you will closely collaborate with hospital discharge teams and care homes to source the best care packages for our clients. About the Role Every day you will… Collaborate with care homes, care agencies, senior health and social care professionals, patients, and their families in Epsom Hospital. Interact with families in a caring and empathetic manner. Act as the liaison between discharge teams, local authorities, clients, and care providers, ensuring timely and high-quality service delivery within the established timeframe.
