At the Birmingham and Solihull Community Care Collaborative, we're looking for reliable, flexible, and proactive individuals to join our Integrated Neighbourhood Teams as Coordinators.

In this role, you'll have the opportunity to work with professionals across health, social care, and the voluntary sector organisations to help deliver more joined-up, personalised care for people within our communities.

You'll play a key part in improving communication, supporting meetings, coordinating actions, and ensuring that information is shared effectively to achieve better outcomes for citizens.

While you will be aligned to a specific Integrated Neighbourhood Team, there is an expectation that you can provide cover for INT colleagues in the same or other Locality when required. You will need to have the flexibility to travel across sites for meetings and events.

Main duties of the job

The INT Coordinator will be a central point of contact and navigator between their designated Primary Care Network (PCN) and the INT multi-disciplinary team (MDT) establishing effective communication processes across primary care, community services, mental health services and the voluntary/third sector.

The INT Coordinator will work in collaboration with the INT multi-disciplinary team and will be responsible for liaising with key workers to manage patient information to support the MDT's triage and MDT meetings including capturing outcomes and evaluation. The INT Coordinator is key to the INT MDT in facilitating the decision-making process.

Person Specification


Qualifications / training

* Educated to GCSE level or equivalent with grade C or above in English and maths
* ECDL or equivalent IT qualification / experience
* IT Systems and project management experience acquired through training or experience to diploma equivalent
* Evidence of continuous personal development


Experience

* Experience of administration procedures
* Experience of working in a customer facing environment, resolving queries and using initiative to ensure effective working relationships across services
* Previous NHS experience or experience of working in a healthcare / social care setting


Skills/Knowledge

* Competent in the use of IT software systems, (including Microsoft Office) and handling sensitive personal identifiable data
* Demonstrable evidence of problem solving and resolving issues as they arise in a fast-changing environment
* Able to communicate clearly and sensitively with a wide range of people, including external partner organisations, community teams and patients and their families
* Excellent written communication skills; able to write clearly and concisely and produce reports and keep accurate records
* Able to induct new staff into a team, providing training to new colleagues
* Adaptable and flexible approach to work with the ability to prioritise workload
* Understanding of the importance of confidentiality and the data protection act
* Self-motivated with the ability to motivate others
* Ability to work autonomously without direct supervision
* Able to prioritise conflicting tasks and demands
* Willing to encourage and drive innovation
* Ability to set high standards for self and others to deliver service priorities


Other job requirements

* Able to travel independently across the Trust as service needs demand
