CLCH Hertfordshire Adult Community Services - CLCH NHS Trust provides a range of Community Services, and the Hertfordshire Division is recruiting.
An exciting opportunity has arisen within our in-patient Intermediate care general rehabilitation service in West Hertfordshire. We are looking for a Band 5 Occupational Therapist to join the OT team and wider MDT at St Peter's Ward, Hemel Hempstead Hospital.
This is a service for the rehabilitation of adults presenting with a range of physical dysfunction/or disabilities resulting from injury, illness, surgery or chronic disease. Many of the patients are referred from an acute hospital, and some may be referred from the community settings and establishments.
Main duties of the job
We are looking for a highly motivated individual to join the team, who has the passion and skills for delivering and maintaining in the team, a high standard of rehabilitation. You will be working with a multidisciplinary team that includes physiotherapists, Nurses, Dietitian, Rehabilitation Assistants, Speech and Language therapists and Social Workers.

Person Specification
Education/Qualification
* Diploma/Degree in Occupational Therapy
* Registration with the Health and Care Professions Council
* Member of RCOT/special interest group
Experience
* Application of the OT Process in a clinical setting
* Use of functional assessment and activity analysis
* Relevant work experience in a variety of clinical placements
* Experience of delegation to support staff
* Experience of working as part of an MDT
Skills & Knowledge
* Knowledge of current best practice in OT
* Ability to analyse, assess and implement treatment plans for a wide range of individuals with rehabilitation needs.
* Ability to carry out cognitive assessments to inform your treatment plan.
* Awareness of how to adapt treatment approach to enhance the rehabilitation experience for those with cognitive impairments.
* Awareness of how to create a safe discharge plan, utilising external agencies to support the clients ongoing rehabilitation and care needs in the community.
* Awareness of prescription of simple aids and equipment to support with enhancing independent functioning.
* Basic knowledge of health legislation and current practice, risk assessment and management
* Understanding of professional ethics and their application in practice
* IT Skills including Word processing and internet skills
* Effective written and oral communication skills
