Responsibilities
* Carry out initial assessments and manage a caseload of older adults with dementia and other mental health conditions.
* Provide professional input to the multidisciplinary team.
* Maintain high standards of documentation.
* Participate in professional supervision and regular CPD sessions.
* Develop skills in assessment, planning and service user management.
* Forge links with the third sector and specialist teams within the Trust.
Qualifications & Experience
* Background in Nursing, Occupational Therapy or Social Work.
* Experience working with older people with dementia and other mental health conditions.
* Empathy, motivation, enthusiasm, creativity and resourcefulness.
* Strong communication skills and ability to develop working relationships with carers.
