Community Staff Nurse (Cockermouth & Maryport)

An exciting Band 5 Community Staff Nurse opportunity has arisen with us in the Maryport Community Nursing Team. The successful candidate will work with our evolving Community Team, as part of the developing Integrated Care Community (ICC) of Maryport and Cockermouth, and will primarily be based with and serve the population of Maryport ICC.
This is a fixed‑term contract for 9 months for 37.5 hours per week, with a view to 12 months.
Main duties of the job
This role requires you to demonstrate the skills and knowledge to support the provision of high‑quality care, to support patients with complex nursing needs within the community setting.
This is an excellent opportunity for dedicated and committed registered nurses who are looking to develop their clinical skills and expertise in caring for patients, working closely with health and social care colleagues to prevent hospital admission, facilitate hospital discharge and promote independence through reablement and the management of long‑term conditions.
We are able to provide an individually tailored preceptorship programme to experienced nurses and also be interested to hear from newly qualified nurses whom we could support in the development of their nursing career.

Job responsibilities
This role will be responsible for delivering high‑quality community nursing care, preventing unnecessary hospital admissions, facilitating patient discharge, and promoting independence through reablement and chronic condition management. Responsibilities include:
* Assessing, planning, and delivering care for patients in the community.
* Working closely with health and social care colleagues to support patient care and discharge planning.
* Providing clinical oversight and support to patients with complex needs.
* Engaging in clinical governance, quality improvement, and professional development activities.
* Participating in multidisciplinary team meetings and care reviews.
Qualifications
* Level 1 registered nursing qualifications
* Post registration qualification in teaching/assessing (ENB 998/NST 307) or equivalent
* Post registration qualification in Acute/Chronic Disease Management
* Nurse Prescribing licence
Experience
* Demonstrable recent community nursing experience.
* Ability to use initiative and prioritise effectively.

PRECEPTISHPERIOD
The Trust will provide a twelve‑month preceptorship period for new registration nurses, midwives and allied health professionals. Registered nurses, midwives and allied health professionals returning to practice, moving into new roles or changing clinical environments, may access an agreed preceptorship period.

£32,073 to £39,043 a year, pro rata
