South AMHT Community Mental Health Nurse

The Wallingford, Henley and Thame AMHT have a rare opportunity for you to join our team on a full time basis.
The team is a busy and dynamic team that holds patient centered-care at the core of what we do. We are currently adapting our service to support co-produced care plans and using Patient Recorded Outcome Measures (PROMs) to identify and guide patient care.
The South AMHT consists of four satellite bases in Wallingford, Abingdon, Didcot and Wantage and consists of the multi-disciplinary team who work alongside Keystone Hubs in primary care and also with embedded third sector services in each locality.

You will be supported by an experienced leadership team from across the professions who hold your development as central. We offer regular supervision and you will be encouraged and supported to undertake any training relevant to your professional development.
Applicants must have a full valid driving licence and have access to a vehicle for work. Adjustments can be considered in line with the Equality Act 2010 where required.
Main duties of the job
The role will involve being the allocated Key Worker (formerly known as Care Coordinator) for a caseload of patients with severe and enduring mental illness. In these instances, you will be:
* Playing an integral role as part of an multi-disciplinary team in that person's recovery, which would include the assessment, collaborative care planning and review of the person's needs.
* Assessing and care planning risk, including understanding and applying safeguarding processes.
* Undertaking assessment of patients newly referred to the team, offering biopsychosocial assessments and providing brief intervention as appropriate.
* Undertake the duty role, which involves the triage of new referrals with the view to signpost people to appropriate services or offer assessments with the team in appropriate timeframes.
You will need to be able to work autonomously, however there will be ongoing support from peers and the leadership team in the form of both informal and formal clinical and management supervision.
The team continues to operate seven days a week, with hours ranging from 07:00-21:00, with core duties are Monday-Friday 09:00-17:00. We are anticipating the implementation of the Crisis Resolution and Home Treatment Team, which will mean that we will then operate Monday - Friday (exclusive of Bank Holidays) 09:00-17:00. This may go live before the successful candidate joins, however we wish to note there may be a possibility of weekend working.

Education and Training
* Professional Qualification (Degree or equivalent)
* Evidence of Continuing Professional Development (CPD) portfolio
* Professional Registration
* Post graduate education specifically relating to client group (Post basic qualification in community care/ research awareness/ management)
Occupational Experience
* To have experience of working within a multidisciplinary team
* To have extensive experience of working with people with severe and enduring mental illness
* Experience working in community setting
Skills required - IT / Admin
* To be IT literate
* To have excellent English written communication skills
