SOCIABLE HOURS - MONDAY TO FRIDAY (9AM TO 5PM)
The first round of interviews will take place via Teams Meeting within two weeks of the closing date.
The Paramedic Practitioner must have at least 5 years' work experience as a Paramedic and will work autonomously both within the community and within the PCN and Practice based multi-disciplinary team. They will use their enhanced clinical assessment and treatment skills to provide chronic care management or first point of contact for patients presenting with undifferentiated, undiagnosed problems relating to minor illness or injury, abdominal pains, chest pains, headache and mental health crisis.
The practitioner will have direct and immediate access when needed to a GP/Doctor and work under supervision of the GP with which the patient is registered.
Main duties of the job
Assess and triage patients, provide definitive treatment ormake necessary referrals
Assess patients presenting with acute illness or for reviewof chronic disease conditions
Undertake home visits for acute illness or chronic diseasemanagement
Possess a valid driving licence
Clinically examine and assess patient needs from aphysiological and psychological perspective and plan clinical care accordingly
Implement and evaluate treatments for patients with undifferentiateddiagnosis / complex needs and follow up
Proactively identify, diagnose and manage treatment plans forpatients at risk of developing a long-term condition
Diagnose and manage both acute and chronic conditions
Advise patients on general healthcare and promoteself-management
Undertake the collection of pathological specimens
Support the delivery of anticipatory care plans
Provide an alternative model to urgent GP home visits
Maintain accurate and contemporaneous healthrecords

Diagnoseand manage both acute and chronic conditions, integrating both drug andnon-drug-based treatment methods into a management plan.
Advisepatients on general healthcare and promote self-management where appropriate,including signposting patients to other community or voluntary services.
Performspecialist health checks and reviews; perform and interpret ECGs; performinvestigatory procedures as required.
Undertakethe collection of pathological specimens including intravenous blood samples,swabs etc.
Supportthe delivery of anticipatory care plans as this process develops (monitoring blood pressure and diabetes risk of elderly patients living insheltered housing).
Providean alternative model to urgent and same day GP home visit.
Maintainaccurate and contemporaneous health records appropriate to the consultation,ensuring accurate completion of all necessary documentation associated withpatient healthcare and registration with the practice.

Forthose patients at risk of hospital admission, for example, in a care home orwith complex needs they have a care plan which is communicated to the out ofhours GP service.

Qualifications
Essential
1. BSc in a training programme approved by the College of Paramedics
2. Health & Care Professions Council (HCPC) registration
3. able to operate at an advanced level of clinical practice
4. has completed the 2 year Consolidation of Learning period as a newly qualified paramedic
5. has a further three years experience as a band 6 (or equivalent) paramedic
6. Framework for Higher Education Qualification (FHEQ) Level 7 or Scottish Credit and Qualifications Framework (SCOF) Level 11
7. ability to write comprehensive clinical notes, implement and evaluate care plans
8. ability to evidence a sound understanding of the NHS principles and values
9. ability to analyse and interpret complex/ often incomplete information, preempt and evaluate issues, and recommend and appropriate course of action to address the issues
10. excellent interpersonal and organisational skills
