We are seeking a dedicated and empathetic Support Worker to join our team at Victoria Close Reading. As a Support Worker, you will play a pivotal role in delivering high-quality personable support to clients in our supported accommodation service, focusing on promoting independence, freedom, and choice. Key Responsibilities: • Engage with Prospective Clients: Utilise a personable approach to assess referrals and engage with prospective clients. Collaborate with health professionals, agencies, and colleagues to ensure comprehensive support. • Inform and Empower: Ensure that clients are fully aware of available support, financial obligations, and their rights and responsibilities under tenancy arrangements. Educate them on related procedures. • Plan and Review: Agree and review Independent Living Plans and Risk Assessments regularly, delivering contracted support hours to achieve desired outcomes. • Flexible Support Delivery: Deliver support as outlined in Independent Living Plans, coordinating additional support from external agencies when needed. • Professional Development: Engage in continuous professional development and reflective practice to enhance skills and knowledge. • Community Engagement: Forge and maintain links with partner agencies, stakeholders, and internal departments for effective communication and cooperation. • Safeguarding: Report concerns related to the safeguarding of vulnerable individuals, attending case conferences and meetings as required. • Documentation and Data Collection: Ensure accurate and timely completion of all relevant documentation. Contribute to data collection for statutory returns. • Empower Clients: Empower clients to maintain project safety, security, and cleanliness, adhering to internal policies and reporting repairs. • Complaint Handling: Handle complaints in accordance with company policy, conducting necessary investigations. • Relationship Skills: Demonstrate high-level relationship skills, including empathy, mutual acceptance, and constructive challenge. • Flexibility: Work outside standard office hours as needed, following lone working procedures. External Contacts & Working Environment: • Engage with Reading Council, Social Services, Police, CMHT, and other support services as needed. • Work with unaccompanied Asylum Seekers at Victoria Close Reading initially (4 beds, potential for expansion). • Flexibility to work across other schemes as needed. Knowledge, Skills, and Competencies: • Working knowledge of supporting people in achieving and maintaining independence. • Educated to GCSE level or equivalent in Maths and English. • Ability to work under own initiative. • Networking and relationship-building skills. • Organisational skills, including time management and meeting targets. • Effective communication at different levels. • Proficiency in using IT packages and management information systems. • Ability to handle distressed and aggressive clients.
