Team Leader - Supported Living (B78DM)


Position: Team Leader - Supported Living

Reporting to: Operational Manager - Supported Living

Contract Type: Permanent, Full Time, 37.5hrs

Salary: 30,000 per annum

Role:

The Team Leader plays a key role in ensuring high-quality, person-centred support across supported living services. They will oversee day-to-day operations including rota coordination, care planning, and direct casework with a defined caseload. The postholder will act as a subject-matter champion in their specialist area (Mental Health, Learning Disabilities, or Substance Misuse) and provide guidance, coaching and professional oversight to staff.

Responsibilities:

Operational Leadership

* Coordinate staff rotas to ensure adequate cover, continuity and cost-efficient deployment.

* Supervise and support Support Workers to deliver safe, person-centred care.

* Provide on-call cover on a rota basis as required.

* Maintain a safe and positive environment aligned with organisational values and regulatory standards.

Case Management & Support Delivery

* Hold a caseload of clients, providing direct keyworking support tailored to assessed needs.

* Develop and maintain high-quality support/care plans and risk assessments, ensuring they reflect individuals goals, preferences and outcomes.

* Undertake regular reviews of support plans with clients, families, commissioners and other agencies.

* Champion best practice in Mental Health, Learning Disabilities or Substance Misuse, offering specialist knowledge and informal training to the team.

* Promote independence, recovery and wellbeing through evidence-based approaches and appropriate goal setting.

Quality & Compliance

* Ensure service delivery aligns with CQC, local authority and commissioning requirements.

* Maintain accurate records including case notes, incident reports and support documentation.

* Support in preparing for inspections, audits and reviews.

* Drive continuous improvement, identifying areas for development and implementing solutions.

Team Development

* Mentor and coach staff; actively support skills development.

* Lead team meetings and reflective practice sessions.

* Support with staff induction, mandatory training compliance, and supervision.

* Role model high standards of professional conduct and person-centred practice.

Partnership Working

* Build positive relationships with families, health professionals, social workers, housing providers and other external partners.

* Advocate for clients, ensuring their voice remains central to all decisions.

* Liaise with commissioners and clinical teams to support placements and positive outcomes.

Qualifications and Skills:

Essential

* Experience supporting people in supported living or similar care settings.

* Experience working with people with Mental Health needs, Learning Disabilities and/or Substance Misuse.

* Strong organisational skills with previous involvement in rota planning or service coordination.

* Experience developing support plans and risk assessments.

* Ability to manage a caseload and prioritise competing demands.

* Strong communication skills - verbal, written, and interpersonal.

* Ability to work flexibly, including occasional evenings/weekends.

* Commitment to safeguarding, dignity, and positive behaviour support.

Desirable

* Psychology degree, health & social care qualification, or equivalent experience.

* Experience delivering specialist support interventions in mental health, LD or substance misuse.

* Experience supervising or mentoring teams.

* Working knowledge of CQC regulations and supported living frameworks.

* Full UK driving license.
