Job summary
One Ellesmere Port Primary Care Network (PCN) is a collaboration of 6 GP Practices across the town, working together to develop better services for approximately 72,000 patients.
Whitby Health Partnership is a core member Practice of the PCN and the second largest Practice.
The successful candidate will be employed by and based at Whitby Health Partnership, focusing on their patient population. They will join a team of 5 Mental Health Occupational Therapists based across other surgeries within the PCN. This team meets regularly for peer support and best practice sharing.
Main duties of the job
As the Mental Health Lead for Whitby Health Partnership, you will provide specialist advice and support to colleagues and patients. This includes first-contact assessments, developing treatment plans, and offering MH OT interventions or signposting to services.
Key responsibilities include:
* Assessing functional capacity for holistic treatment
* Supporting recovery and independent living skills
* Managing symptoms and coping strategies
* Liaising with GPs and mental health teams
* Navigating referrals between primary and secondary care
The role bridges IAPT and secondary care, managing a hybrid model of in-person and telephone appointments, including urgent assessments. The candidate will manage their caseload independently with team support.
