A fantastic opportunity has arisen for a Band 6 Physiotherapist to join our Whitby Community Therapy Team.

This role offers the chance to work in one of the most diverse and rewarding community settings in the region. You'll provide specialist physiotherapy to people living in Whitby and the surrounding villages, supporting them to stay well, remain independent, and recover in the comfort of their own homes.

You will play a key role in our Urgent Crisis Response and intermediate care pathways, delivering timely assessments, rehabilitation, and preventative interventions. You'll also support junior colleagues, mentor students, and contribute to ongoing service development.

We're looking for a physiotherapist who thrives in a community environment, enjoys the autonomy and variety it brings, and is motivated by helping people live well at home and committed to delivering excellent patient outcomes. Main duties of the job

The main duties of the role will include:

* Triage & prioritise new referrals received by the service
* Responsibility for a designated caseload of patients: to organise this effectively and efficiently with regards to clinical priorities and use of time
* To undertake a comprehensive assessment of patients including those with diverse or complex presentations using clinical reasoning skills
* Support patients with individualised goal setting, education and care plans to work towards fulfilling their potential in a targeted and realistic way via delivery of evidence based clinical interventions and practice in partnership with patients, relatives, staff, managers, and externally with other agencies
* To be accountable in own area of clinical responsibility participating in regular clinical and line management supervision, completing all required training and ensuring own continued professional development
* Have a Home first ethos and outreach into community for patients recently discharged from rehabilitation inpatient unit.
* be on a rota system to undertake UCR and Intermediate care role as part of a community role with a caseload.

Person Specification


Experience

* Evidence of sound post-registration professional practice
* Evidence of continuing professional development (CPD) relevant to the clinical area at a specialist level of care
* Experience of complex case load management, within specialist area
* Experience or working in Community based physiotherapy
* Experience of using SystmOne


Skills and Competencies

* Can commute effectively to facilitate community based visits including transportation of equipment required for clinical interventions
* IT skills to facilitate use of electronic patient records & other electronic systems e.g. for audit and equipment ordering
* Effective inter-personal skills and experience in supervising and mentoring
* Able to demonstrate effective communication skills and work flexibly within the team/service
* Ability to carry out basic observations and calculate NEWS2 score
* Ability to identify and set up home based assistive equipment according to individual patient need


Qualifications and Knowledge

* Degree/ Diploma in Physiotherapy
* Current HPC professional registration
* Professional knowledge acquired through degree supplemented by specialist training, experience in specialist field(s) to post-
* Full understanding of relevant policy, legislation and guidance relevant to clinical and service area
* Specialist understanding of relevant clinical practice, standards and protocols within identified clinical area
* Member of specialist interest group/network, locally or regionally
