Outreach Support Worker Complex Needs Support Package
We are seeking compassionate, resilient, and motivated Outreach Support Workers to join a brand-new domiciliary support package.
This is an exciting opportunity to make a genuine difference in the life of an individual with complex support needs, helping them to live as independently as possible within their community.
As an Outreach Support Worker, you will provide person-centred support tailored to the individual's unique needs, preferences, and goals. You will build positive, trusting relationships while promoting independence, choice, dignity, and wellbeing.
The role may involve supporting an individual with learning disabilities, autism, mental health needs, behaviours of concern, physical health needs, or communication difficulties.
You will be required to follow structured support plans, risk assessments, and positive behaviour support strategies to ensure safe and effective support is delivered at all times.
Key responsibilities include:
• Providing high-quality, person-centred support in line with individual support plans.
• Promoting independence and encouraging the development of daily living skills.
• Supporting access to community activities, appointments, education, employment, and social opportunities.
• Assisting with personal care, medication administration, and health-related needs where required.
• Implementing positive behaviour support approaches and maintaining consistent routines.
• Accurately recording support provided and reporting any concerns.
• Working collaboratively with families, healthcare professionals, and multidisciplinary teams.
• Responding calmly and professionally in challenging situations while maintaining the individual's dignity and safety.
The ideal candidate will be patient, reliable, adaptable, and committed to empowering people to achieve positive outcomes.
Experience supporting individuals with complex needs is desirable, although full training will be provided to the right candidates.
