What You'll Do:
1. Provide tailored care in both day service and community settings, responding to complex and evolving needs.
2. Maintain accurate and auditable care records including daily notes, risk assessments, and incident reports.
3. Act as a keyworker for individuals, contributing to care plan reviews and wellbeing reports.
4. Recognise and escalate changes in health or behaviour to ensure timely interventions.
5. Deliver personal care with dignity and respect - including bathing, dressing, toileting, and mobility support.
6. Administer prescribed medication safely and accurately, including specialist preparations.
7. Foster meaningful communication and emotional wellbeing through social engagement and activities.
8. Respond swiftly and effectively in emergency situations, including administering first aid and rescue medication.
9. Collaborate with families, carers, and professionals to ensure holistic, person-centred care.
What We're Looking For:
10. Experience in care or support work (preferred but not essential - training provided).
11. Compassionate, reliable, and proactive individuals.
12. Strong communication and record-keeping skills.
13. Ability to work independently and as part of a team.
14. Willingness to undertake personal care and medication administration.
