As a key member of the PCN team, you will use your clinical expertise to conduct comprehensive medication reviews, support chronic disease management, and lead medicines optimisation initiatives. Main duties of the job

The role involves delivering high-quality, patient-centred pharmaceutical care through comprehensive clinical assessments, medication reviews, and evidence-based prescribing. It includes managing complex medication regimens for patients with chronic conditions, leading medicines optimisation initiatives, and ensuring safe, effective prescribing aligned with local guidelines. The postholder collaborates closely with multidisciplinary teams, supports quality improvement projects, and contributes to enhanced services and antimicrobial stewardship. Ongoing professional development, supervision, and mentoring of pharmacy colleagues are also integral to the role.


About us

Fleetwood Primary Care Network in Fleetwood, Lancashire has an exciting opportunity for a forward-thinking Clinical Pharmacist to join our network's well established pharmacy team. There are 3 surgeries within the PCN, located within close proximity of each other, with excellent inter-personal relations, a good support network and a history of collaborative working. The practices are The Mount View Practice, Broadway Medical Centre and Fleetwood Surgery. Our population of around 32,000 is diverse, with varying levels of socio-economic status, young families and elderly patients all of whom make providing care very interesting.


Job responsibilities

Key Accountabilities Patient Care and Clinical Assessment

* Conduct comprehensive clinical assessments of patients with complex medication needs
* Provide patient-facing consultations using expert knowledge of medicines for specific disease areas
* Deliver evidence-based pharmaceutical care tailored to individual patient needs
* Assess medication-related problems and provide appropriate interventions

Prescribing and Medication Management

* Act as an independent prescriber (if qualified) working with and alongside the general practice team
* Prescribe medicines within scope of practice and competence (qualified prescribers only)
* Work collaboratively with GPs and practice team to support medication decisions
* Provide clinical recommendations for medication therapy
* Review and advise on medication regimens and therapeutic options
* Ensure prescribing practices and recommendations align with local guidelines and formularies

Chronic Disease Management

* Manage patients with chronic conditions requiring complex medication regimens
* Conduct structured clinical medication reviews, particularly for:
* Complex polypharmacy cases
* Older people and care home residents
* Patients with multiple co-morbidities (frailty, COPD, asthma)
* People with learning disabilities or autism (STOMP programme)
* Monitor treatment outcomes and adjust therapy as appropriate
* Lead medicines optimisation initiatives across the practice
* Provide specialist expertise in safe and effective medicines use
* Address medication-related health inequalities
* Support patients in understanding their medications and improving adherence
* Reduce medication waste through appropriate review and deprescribing

Quality Improvement and Leadership

* Lead quality improvement projects related to pharmaceutical care
* Contribute to LES contract achievements
* Implement antimicrobial stewardship programmes
* Develop and maintain clinical protocols and guidelines

Professional Collaboration and Integration

* Work effectively within multidisciplinary primary care teams
* Collaborate with community and hospital pharmacy services
* Develop professional relationships across the PCN and wider healthcare system
* Participate in shared care protocols and specialist referral pathways

Professional Development and Supervision

* Participate in regular clinical supervision and peer support networks
* Maintain continuing professional development in line with GPhC requirements
* Contribute to training and mentoring of pharmacy colleagues
* Stay current with evidence-based practice and emerging therapies

Key Performance Indicators

* Clinical outcome measures for managed conditions
* Prescribing quality indicators and cost-effectiveness measures (if applicable to qualified prescribers)
* Medication therapy recommendations quality and acceptance rates
* LES and enhanced services contributions
* Professional development and training completion
* Integration with practice workflow and team effectiveness


Person Specification


Qualifications

* Registered Pharmacist with the General Pharmaceutical Council GPhC.
* Completed or willing to undertake an approved 18-month training pathway (such as CPPE Clinical Pharmacist pathways) that equips you to practice safely in primary care.
* Independent Prescriber Experience in the Primary Care environment.


Experience

* Excellent interpersonal and communication skills.
* Computer literacy and experience of Primary Care Medical IT systems ie; EMIS.
* Clinical experience in a healthcare setting.
* Strong communication and interpersonal skills.
* Ability to work independently and as part of a multidisciplinary team
* Commitment to continuing professional development and clinical supervision.
* Previous experience working in general practice or primary care
* Experience with clinical medication reviews and chronic disease management
* Knowledge of local prescribing guidelines, formularies, and antimicrobial stewardship
